In adherence with Board Rule 1.17 and HB 25-1024, our spa and its Medical Director, Michael R. Iannotti, MD, are offering medical-grade aesthetic services delegated from a physician to licensed and unlicensed persons/delegatees who are trained and certified.
This notice is to inform you of the following:
Michael R. Iannotti, MD, is our Medical Director. He is board certified in Family Medicine and extensively trained in medical-grade aesthetic consultations and procedures.
Dr. Iannotti delegates his licensure and authority to practice to the contractors and employees of this aesthetic spa, both licensed and unlicensed, so they may provide services to patients. These individuals are trained, certified, and delegated authority by a licensed physician.
Any delegatee who practices outside of his or her scope of practice does so under Dr. Iannotti’s authority and with training and advanced certification from third-party agencies.
Dr. Iannotti is available for pre-procedure advice, consultation, post-procedure advice, complication treatment, remedies, and consultation.

A copy of this information is available on this website.
A copy of this informed consent is posted in the reception area of the spa and maintained with the patient’s medical chart for seven (7) years. After seven years, patient charts may be destroyed in accordance with state law.
